In claims involving treatment of more than one condition or
more than one part of the body, the attending physician must report all
conditions and all parts of the body being treated that are affecting the
length of disability. The conditions submitted shall include a primary
international classification of disease diagnosis code and a description of the
condition being treated. This information may be used in the determination of
the extent of disability resulting from the industrial injury or occupational
disease.
