Chapter4501-54. Motor Vehicle Collision Prevention Course |
4501-54-08. Course records and insurance

(A) The course instructor shall maintain all course records in a place as designated on the "Mature Driver Course Provider Application." All records shall be kept for a three-year period.
(B) The course instructor shall maintain the "Certificate of Operation."
(C) The course instructor shall maintain the "Mature Operator Course Place of Business Supplement" for each location used for training. The records shall include the business name and address of any mobile ranges used for this course.
(D) The record of the evaluation of the skills demonstration shall include the following information:
(1) Date of training;
(2) Name of the business where training took place;
(3) Address of the business where training took place;
(4) Contact information of the business where training took place;
(5) Address of the range if different from where training took place.
(E) The course records shall be open to inspection by the director of the department of public safety or a designated representative during normal business hours.
(F) The enterprise shall maintain financial responsibility for the operation of all motor vehicles used for instruction as specified in Chapter 4509. of the Revised Code. Such insurance coverage shall be acquired from a company licensed to do business in this state and shall be maintained as long as the enterprise is operating as a mature operator program. Proof of this insurance coverage shall be maintained in the school office. The certificate of insurance shall be available for inspection during normal business hours.
(1) Insurance coverage shall be maintained in the following minimum limits:
(a) Five hundred thousand dollars combined single limit liability coverage; and
(b) Five thousand dollars medical payments for each occupant; and
(c) Uninsured/underinsured motorists coverage at least equal to the bodily injury coverage required in paragraph (G)(1)(a) of this rule; or
(d) Fifty thousand dollars for injury to property of others in any one crash; or
(e) One hundred thousand dollars for bodily injury to or death of one person in any one crash; and
(f) Three hundred thousand dollars for bodily injury to or death of two or more persons in any one crash; and
(g) Five thousand dollars medical payments for each occupant; and
(h) Uninsured/underinsured motorist coverage at least equal to the bodily injury coverage required in paragraphs (G)(1)(e) and (G)(1)(f) of this rule.
(2) Failure to maintain the required insurance, including record of current coverage as required by this rule, may result in the suspension or revocation of an authorizing official's mature operator course approval.
(G) Participants providing motor vehicles for their own instruction, before such motor vehicle is used for any instruction, shall provide proof of insurance or other financial responsibility that indicates compliance with Chapter 4509. of the Revised Code.
