4713-7-01. Processing of applications for licensing and examination

  • All applications for licensing and examination for any branch of cosmetology shall be made upon forms supplied by the board in paper or electronic format. All statements made on the application form shall be subscribed and sworn to by the applicant and attested to under oath by signature of the applicant. For the purposes of filing an electronic application with the Ohio elicensing system, the applicant shall create and maintain a registered "User ID" and password for the system. The use of the "User ID" password created by the applicant is the sole responsibility of the applicant or licensee to secure and maintain, and use shall be limited to filing an electronic application for initial licensure and examination and subsequent renewals of such licenses. The "User ID" and password shall constitute the legally recognized signature of applicant or licensee for the purpose of this rule and may not be transferred, distributed, or shared with any other person. After an application for examination is approved, an applicant will receive a date upon which he or she is scheduled to take the examination. All application fees are non-refundable.
