4725-3-22. Reinstatement of inactive status certificate of licensure, delinquent certificate of licensure fees

Latest version.

Inactive status licensure reinstatement fees are seventy-five dollars, and delinquent status licensure reinstatement fees are seventy-five dollars, for each year that the certificate is classified as delinquent. All fees are nonrefundable.
