    In reviewing the rule and the associated appendices during the five-year review process, ODH realized that Appendix M had been accidentally omitted from the previous filing. In addition, ODH proposes to deleted reference in the text of the rule to an obsolete appendix and correspondingly changed the lettering.

    This rule lists the forms used by the Ohio Department of Health Office of Vital Statistics. ODH is amending the rule to include an atachment (Appendix M), which was omitted in the previous rule filing. In the text of the rule, ODH also removed reference to a "Certification of Death" form labeled Appendix EE because the form is obsolete and was not filed with the rule in 2005; also, Appendix FF was changed to EE to correctly correspond to the Certification of Stillbirth as previously filed.
