145-1-71                      Withdrawal of retirement application.

 

 

 

(A)    A member of the public employees retirement system may withdraw a retirement application by either of the following methods:

 

(1)   By returning Returning to the retirement system not later than thirty days after issuance of the first initial retirement warrant, all uncashed warrants, along with a written request over the member's signature to withdraw the application; or,

 

(2)    By remitting Remitting to the retirement system a personal check or money order repaying the retirement benefit(s) transmitted by the retirement system to the member's financial institution not later than thirty days after the institution's receipt of the first initial retirement benefit along with a written request over the member's signature to withdraw the application.

 

(B)    A member of the retirement system may withdraw an application for a disability benefit by either of the following methods:

 

(1)   By returning Returning to the retirement system not later than thirty days after issuance of the first initial disability warrant, all uncashed warrants along with a written request over the member's signature to withdraw the application; or,

 

(2)    By remitting Remitting to the retirement system a personal check or money order repaying the disability benefits benefit(s) transmitted by the retirement system to the member's financial institution not later than thirty days after the institution's receipt of the first initial disability benefit along with a written request over the member's signature to withdraw the application.
