5101:2-13-06 Procedures for renewal of a type A home license.
(A)
The following must be completed by the person responsible for the renewal of alicense:The person responsible for the renewal of a license shall complete and submit the JFS 01210 "Application for Child Care License" (rev. 7/2010) and all other required forms along with payment of a license renewal fee to the Ohio department of job and family services (ODJFS) at least sixty calendar days prior to the expiration date of the current license.(1)The application shall be considered filed with ODJFS as of the date it is postmarked or received electronically.
(2)An application received by means other than the postal service shall be considered filed when it is time/date stamped by ODJFS.
(3)Any application submitted without complete and/or accurate information must be amended with complete and/or accurate information before a license can be issued.
(1) Complete and submit the JFS 01210 "Application for Child Care License" (rev.8/2008) and all other required forms, and payment of a license renewal fee offifty dollars at least sixty calendar days prior to the expiration date of thecurrent license.(2)(4)Send the JFS 01210 to the Ohio department of job and family services(ODJFS) not less than sixty days prior to the expiration of the type A home'scurrent license. The application shall be considered filed with ODJFS as ofthe date it is postmarked. An application received by means other than thepostal service shall be considered filed when it is time/date stamped inODJFS's revenues and collections unit of child care licensing. Anyapplication submitted without complete and/or accurate information must beamended with complete and/or accurate information.(B) When ODJFS does not receive the application and fee at least sixty days prior to the expiration date of the current license, the license shall not be renewed and the person responsible for renewal of a license shall complete and submit the JFS 01210 for an initial application. An initial license shall not be issued until all requirements for an initial application have been met.
(C) Failure to file an application for renewal of a license prior to the expiration date of the current license renders the current license invalid as of the license expiration date. The type A home:
(1) Loses all legal rights to continue to do business at the licensed location after the expiration date of the current license if the application and license renewal fee
is not filed with ODJFS in compliance with the requirements of this rule.
(2) Has no rights to an appeal through the administrative hearing process pursuant to section 5104.03 and Chapter 119. of the Revised Code.
(3) Shall be subject, after the license expiration date, to any penalty that could be assessed under section 5104.99 of the Revised Code.
(D) After the application for renewal is submitted the type A home shall receive an inspection by ODJFS for compliance with Chapter 5104. of the Revised Code and Chapter 5101:2-13 of the Administrative Code. The type A home shall correct any noncompliances in the written inspection report by the date noted in the report and submit all documentation requested in the inspection report.
(E) If the type A home meets all of the requirements of Chapter 5104. of the Revised Code and Chapter 5101:2-13 of the Administrative Code ODJFS shall renew the license for a period of two years.
