145-2-27 Disability benefit recipient's annual statement.
(A)
On(1)On or before April fifteenth of each year, a disability benefit recipient shall file a statement with the public employees retirement system providing information including, but not limited to: work performed during the preceding calendar year, compensation received for work performed, and current medical information.
(2)For disability benefit applications filed on or after January 7, 2013, the statement must also include any social security disability insurance ("SSDI") benefit payments and include a copy of the annual SSDI reward letter.
(B) The recipient's statement shall be made on a form provided by the retirement system.
(C) The failure of a recipient to file such statement shall result in the suspension of a disability benefit until such statement is filed. If such failure continues for one year, the disability benefit shall be terminated.
