    5101:2-14-24               Parent  Caretaker/provider  responsibilities  for  professional certification as a type B home provider or in-home aide.

     

     

     

    (A)   The provider shall conduct an interview with the parent caretaker and the child at the type B provider's home or the home of a child receiving in-home aide services prior to the child's receipt of child care services. the The type B home provider shall provide the parent caretaker with the opportunity to view all areas of the home where child care will be provided. The type B home provider or in-home aide shall provide the parent caretaker with the opportunity to exchange information and to arrive at agreed-upon decisions regarding the care of the child.

     

    (B)    During the interview, the parent caretaker and provider shall jointly discuss and complete a written agreement using the ODHS 1634 JFS 01634 "Parent Caretaker/Provider Agreement".

     

    (1)  The written agreement shall include, but not be limited to, the following:

     

    (a)   Meals and snacks to be served;

     

    (b)   Provider's policies for emergencies, accidents, illnesses, administration of medications, modified diets, and food supplements;

     

    (c)   Method of discipline;

     

    (d)  Permission to transport a child;

     

    (e)      Permission  for  the  child  to  participate  in  water  play  or  swimming activities;

     

    (f)  Permission for release of the child;

     

    (g)   Parental Caretaker visitation which shall include the following statements.

     

    (i)    The parent caretaker has unlimited access to all areas where child care is provided during the hours of operation; and

     

    (ii)   The parent caretaker will notify the type B provider that he is on the premises before entering the type B provider's home.

     

    (h)   Provision of infant care supplies;

     

    (i)   Established rate to be paid and fee payment arrangements;

     

     

    (j)   Alternate care arrangements during provider's absence;

    (k)   Termination notice for parents the caretaker and provider;

    (l)   Napping and sleeping arrangements; and

    (m)     A plan for the care of a child with handicapping conditions, when applicable.

    (n)   Number and ages of children to receive in-home aide services.

    (o)     Whether or not the in-home aide's children (maximum of two) will accompany him/her to the home of the child receiving in-home aide services.

    (p)    Who will provide the food served to the in-home aide or the in-home aide's children that may accompany the in-home aide to the home of the child receiving in-home aide services.

    (q)   Days and hours that child care is to be provided.

    (2)     The provider shall give the completed agreement to the parent caretaker immediately and to the CDHS CDJFS within five days of completion.

    (3)   The agreement shall be amended by participating parties as needed.

    (4)   The provider shall give a copy of the amended agreement to the parent caretaker immediately and to the CDHS CDJFS within five days of amendment for approval.

    (C)   The provider shall not impose on the parent caretaker any charge which exceeds that established by the CDHS CDJFS for basic publicly funded child care services.

    (1)    The provider may, with approval from the CDHS CDJFS, establish fees to recover from the parent caretaker costs incurred due to the following:

    (a)   Late arrival of the parent caretaker for pick-up of the child which causes the provider to deliver additional hours of service beyond what has been authorized by the CDHS CDJFS; or

    (b)   Additional cost to the provider for special events and field trips such as, but not limited to, admission fees for movies, plays, and museums. ; or

    (c) Absentee days which exceed those reimbursed by the CDJFS.

    (2)     Extra fees charged to the parent caretaker must be reflected in the parent caretaker/provider agreement and a copy forwarded to the CDHS CDJFS for approval.

    (D)     If the provider or parent caretaker determines that a conference is needed, the provider shall schedule a conference at a time mutually acceptable to discuss the child's progress and needs.
