3701-83-32 Quality assessment and performance improvement - inpatient rehabilitation facilities.
As part of the quality assessment and performance improvement
systemprogram required under rule 3701-83-12 of the Administrative Code, each inpatient rehabilitation facility shall collect and maintain the following data on an annual basis and shall report such data to the director upon request:(A) The total number of patients admitted to the facility;
(B)
The
total number of patients discharged from the facility:;(C)
The total
number of patients
transferred to an acute care setting, such as, but not
limited to, a hospital;(D)
The total number of patients transferred to a long-term
care setting, such as, but not
limited to, a nursing home;(E) The total number of patients who achieved discharge goals; and
(F) The total number of patients readmitted to the facility for rehabilitation needs based on the same episode.
