    As a result of our 5 year rule review process and to update rules affecting the ODMH certification process.

    This rule requires each agency to have written policies and procedures for the

    development on an individualized service plan (ISP) for each person receiving services. It state the minimum requirements for the content of an ISP. It outlines timeframes for completion and review of ISPs. It specifies content requirements for progress notes which reflect ISP implementation. It also outlines requirements for documentation if a person is terminated involuntarily from a service or from the agency.
