    ODH has reviewed Chapter 3701-5 rules pursuant to FYR requirements and is proposing rule 3701-5-02 with amendments.

    The rule lists the forms as used in Chapter 3701-5. Correction has been made to some form titles and is proposing removal of the following obsolete forms: Provisional Death Certificate, Supplemental Form of Given Name of Child, Authorization to Release Information about Biological Parent, Authorization to Release Information about Biological Sibling, Withdrawal of Authority to Release Information about Biological Parent, Withdrawal of Authority to Release Information about Biological Sibling, Disclosure Statement for Medical and Health Information, and Request for Assistance by Adopted Person. Forms relating to a child's birth record have been modified so parents can self-select parent identifiers based on the Obergefell v. Hodges ruling. Forms relating to marriage and divorce have been amended to reflect gender neutral language.

    Section (B) of the rule amends the statement that signatures shall be handwritten in black ink or by electronic signature, and removes the requirement that the printed name shall reside under the signature.

    5/23/2016 - OAC 3701-5-02 and 3701-5-07 are further amended to substitute "fetal death certificates" for "fetal death reports" to be consistent with earlier changes.
