5101:2-5-22 Recommendations for initial foster home certification.
(A) An agency shall not recommend for certification any applicant whose residence or person is licensed, regulated, approved, or operated under the direction of, or otherwise certified as a facility to provide overnight substitute care for unrelated persons, by the department of youth services, the department of mental health, the department of alcohol and drug addiction services, a community alcohol, drug addiction and mental health board, the department of
mental retardation anddevelopmental disabilities, a county board of mental retardation and developmental disabilities, the department of health or a juvenile court.(B)
For each initial application for a foster home certificate submitted to an agency andapproved through an assessor's assessment of the residence and prospective fostercaregiver using either the JFS 01673 "Ohio Department of Job and Family ServicesAssessment for Child Placement (Homestudy)" (rev. 8/2005) or, until December31, 2006, the JFS 01349 "Foster Home Homestudy" (rev. 1/2003), an agency shallsubmit to the Ohio department of job and family services (ODJFS) a JFS 01317"Recommendation for Certification/Recertification of a Foster Home" (rev. 1/2003)to make the recommendation for certification.(B)Each initial application for certification of a foster home shall be approved through an assessor's assessment of the residence, prospective foster caregiver, and household members.
(C)To submit a recommendation for certification of a foster home:
(1)A private child placing agency (PCPA) or private noncustodial agency (PNA) shall submit to the Ohio department of job and family services (ODJFS) a JFS 01317 "Recommendation for Certification/Recertification of a Foster Home" (rev. 1/2003).
(2)A public children services agency (PCSA) shall enter the information into the statewide automated child welfare information system (SACWIS) as required in rule 5101:2-33-70 of the Administrative Code.
(C)Not later than the date the agency submits the JFS 01317 to ODJFS, the agency shallalso submit the necessary data to JFS to register the foster home in the ODJFSuniform statewide automated child welfare database.(D)Upon receipt of a JFS 01317, ODJFS will determine the effective and expiration dates of the certificate., and enter the necessary data into the ODJFS uniformstatewide automated child welfare database to generate the foster home certificate.(E)The effective date of an initial foster home certificate shall be no earlier than the day the JFS 01317 is received by ODJFS as evidenced by the receipt date stamp or the date the recommendation is routed to ODJFS in SACWIS.(F)Effective January 1, 2007 theThe JFS 01673 "Assessment of Child Placement" (rev. 8/2005) shall be used to initiate all foster home homestudies.Homestudies usingthe JFS 01349 that are started prior to December 31, 2006 may be completed usingthe JFS 01349.Effective: 08/01/2010
