4755-7-11 Limited permit holders.
(A)Definitions.
(1)"Limited permit holder occupational therapist" means a person who holds a limited permit to practice as an occupational therapist under the supervision of a licensed occupational therapist.
(2)"Limited permit holder occupational therapy assistant" means a person who holds a limited permit to practice as an occupational therapy assistant under the supervision of a licensed occupational therapist.
(B)Roles and responsibilities.
(1)The limited permit holder occupational therapist may perform the following under the supervision of the supervising occupational therapist, as defined in paragraph (F) of rule 4755-7-01 of the Administrative Code.
(a) Interpretation of referrals for occupational therapy services;
(b) Interpretation and analysis for evaluation purposes; and
(c) Development, interpretation, and modification of the treatment/intervention plan and the discharge plan.
(2)The limited permit holder occupational therapy assistant may contribute to and collaborate in:
(a) The evaluation process by gathering data, administering standardized tests and/or objective measurement tools, and reporting observations;
(b) The preparation, implementation, and documentation of the treatment/intervention plan and the discharge plan; and
(c) Choosing the appropriate treatment interventions.
(3) The limited permit holder occupational therapy assistant may independently:
(a) Select the daily modality of choice according to the established treatment/intervention plan.
(b) Document the progress and outcomes summary.
(4)The limited permit holder occupational therapy assistant may not evaluate independently or initiate treatment/intervention before the supervising occupational therapist performs an evaluation.
(C)Delegation.
(1) The limited permit holder occupational therapy assistant may implement the occupational therapy treatment/intervention plan established by the supervising occupational therapist, as defined in paragraph (F) of rule 4755-7-01 of the Administrative Code. The supervising occupational therapist shall consider the following when delegating to the limited permit holder occupational therapy assistant:
(a) The clinical complexity of the client;
(b) The competency of the limited permit holder occupational therapy assistant;
(c) The limited permit holder occupational therapy assistant's level of training in the treatment/intervention technique; and
(d)Whether continual reassessment of the client's status is needed during treatment/intervention.
(2) Notwithstanding paragraph (C)(1) of this rule, the limited permit holder occupational therapy assistant may respond to acute changes in the client's condition that warrant immediate action.
(D)Supervision.
(1) Supervision must ensure consumer protection. The supervising occupational therapist, as defined in paragraph (F) of rule 4755-7-01 of the Administrative Code, is ultimately responsible for all clients and is accountable and responsible at all times for the actions of limited permit holders supervised.
(2) Any limited permit holder supervised by an occupational therapist shall be included when determining the occupational therapist/occupational therapy assistant supervisory ratios established in paragraph (B) of rule 4755-7-04 of the Administrative Code.
(3) Supervision of the limited permit holder requires initial direction and periodic inspection of the service delivery and relevant in-service training. The supervising occupational therapist need not be on-site, but must be available for consultation with the limited permit holder at all times.
(a) The supervising occupational therapist must provide in person supervision at least one time per week.
(b) The supervising occupational therapist shall consult with the limited permit holder before the limited permit holder initiates any of the client's treatment/intervention plan and/or modifies the treatment/intervention plan.
(c) Supervision requires an interactive process between the supervising occupational therapist and the limited permit holder. The interactive process must include, but is not limited to, review of the following:
(i) Client assessment;
(ii) Client reassessment;
(iii) Treatment/intervention plan;
(iv) Intervention; and
(v) Discontinuation of treatment/intervention plan.
(d)Co-signing client documentation alone does not meet the minimum level of supervision.
(e) It is the responsibility of the occupational therapy practitioner to establish evidence that the supervision occurred in accordance with the requirements of this rule and rule 4755-7-04 of the Administrative Code. This evidence may include documentation in the client record, or it may exist as a separate document, such as a collaboration log.
(E)Documentation.
All limited permit holders shall use the following credential following their signature to indicate a current limited permit:
(1) "LP/OT" for limited permit holder occupational therapists.
(2) "LP/OTA" for limited permit holder occupational therapy assistants.
