This rule is being rescinded and the language is found in new rule 5160:1-2-15 as part of a five year review.

This rule establishes the responsibilities of the County Departments of Jobs and Family Services (CDJFS) in administering Healthchek, Ohio's early and periodic screening, diagnostic, and treatment (EPSDT) program. The rule also requires that child custodial agencies, such as the public children services agency (PCSA), private child placing agency (PCPA), and Title IV-E agencies, inform substitute caregivers of and submit documents concerning Healthchek to CDJFS.

This rule required PCSA, PCPA and Title-IV-E agencies to inform substitute caregivers of Healthchek services and have these caregivers complete form JFS 03528. These agencies were then required to submit the JFS 03528 to the Administrative Agency to achieve compliance.
