    NOTICE OF PUBLIC HEARING

     

     

     

    The Director of the Ohio Department of Health has proposed to amend existing rules 3701-5-01, 3701-5-02, 3701-5-04, and 3701-5-05; and to propose new rule 3701-5-16 of the Ohio Administrative Code (O.A.C.). These rules pertain to Vital Statistics, and they have been reviewed pursuant to R.C. 119.032 (five -year rule review). Amendments to the rules are being proposed for the purpose of clarity and to add new and delete obsolete forms. New rule 3701-5- 16 is being proposed pursuant to R.C. 3705.09(C) to address out of institutional births, and to provide instructions on what is acceptable in registering these births.

     

    In accordance with Chapter 119 of the Revised Code, the Director, or his designee, will conduct a public hearing on the proposed action on Wednesday, October 5, 2005 at 2:00 p.m. The hearing will be conducted in the 8th Floor Conference Room of the Ohio Department of Health, 246 North High Street, Columbus, Ohio.

     

    At the hearing, persons affected by the proposed action may appear and be heard in person, by their attorney or both; may present their positions, arguments, or contentions orally or in writing; may offer witnesses; and may present evidence tending to show that the proposed rules, if adopted or effectuated, will be unreasonable or unlawful. Persons intending to testify are encouraged to pre-register by writing Kaye Norton, Office of the General Counsel, Ohio Department of Health, P.O.
