4715-9-04                    Dental hygiene teaching certificate.

 

 

An application for a dental hygiene teaching certificate must be certified by the administrator of the dental hygiene school or program where the person is authorized to teach. The application shall contain a statement signed by the applicant as to histheir knowledge of the dental laws of the state.
